Join Us as a Case Manager – Make a Real Impact in Denver
We’re looking for a dedicated Case Manager to help individuals thrive across the Denver Metro area. This full-time (40 hrs/week) role offers remote flexibility with 20–30 in-person client visits per month, starting at $22.07/hr with potential to increase to $24.52/hr within six months based on performance.
What You’ll Do:
- Conduct client assessments and create individualized care plans
- Connect clients with services like housing, healthcare, education, and employment
- Monitor progress, update plans, and provide crisis support as needed
- Maintain accurate, confidential documentation
- Collaborate with community partners to enhance service delivery
- Stay current on regulations and best practices
What You Bring:
- Bachelor’s in social work or related field or 5+ years of relevant experience
- Strong interpersonal and organizational skills
- Proficiency in Microsoft Office and electronic records
- Valid driver’s license, clean MVR, and reliable transportation
- Must reside in Colorado
