Lakeshore Talent is seeking a dedicated Physical Therapist (PT) to join an innovative healthcare organization in Albuquerque, NM. This is a full-time, direct-hire opportunity focusing on elderly care within a dynamic, team-oriented environment. Position Details:
- Location: Albuquerque, NM
- Salary: $96,000 – $114,000 ($46.00 – $55.00 per hour)
- Schedule: Monday – Friday | 8:00 AM – 4:30 PM | 40 hours/week
- Work Environment: 100% full-time, elderly-focused care
- Start Date: ASAP
- Software: Epic EMR (Training included)
- Training: 1-month training, including Epic EMR
Required Qualifications:
- Education: Bachelor’s Degree in Physical Therapy
- Accreditation: Completion of a physical therapy program accredited by the APTA, the Committee on Allied Health Education and Accreditation of the AMA & APTA, or an equivalent organization approved by CMS
- Licensure: Active Physical Therapist License issued by the state of New Mexico
- Experience: Minimum 1 year of experience working with frail or elderly populations
- Certifications: CPR/BLS Certification and First Aid (ARC or AHA)
- Other Requirements: Reliable vehicle and valid driver’s license
Preferred Qualifications:
- Experience: 2+ years as a practicing PT
- Education: Master’s Degree (MSPT) or Doctorate of Physical Therapy (DPT)
- Language Skills: Bilingual (Spanish) is a plus

Key Responsibilities:
- Conduct clinical evaluations & treatment plans for elderly participants on an acute, restorative, or maintenance level
- Integrate physical therapy treatment plans into the Plan of Care (POC) per physician orders
- Collaborate with nursing homes, assisted living staff, and interdisciplinary teams regarding rehabilitation needs
- Refer participants to Occupational Therapy (OT) & Speech Therapy (ST) as needed
- Communicate with families and caregivers to ensure adherence to therapy plans
- Provide physical therapy treatments, including modality treatment, therapeutic exercise, and functional mobility training
- Instruct participants and caregivers on proper use of Durable Medical Equipment (DME)
- Conduct home visits and educational sessions as needed
- Participate in quality management activities, peer reviews, and interdisciplinary team meetings
- Ensure proper ordering, distribution, and maintenance of DME, handling replacements and repairs
- Provide training to non-rehab staff (e.g., CNAs, drivers) regarding participant mobility needs
